The Power List: 7 Things We Learned from McCall McPherson
1) TSH alone is not thyroid care
McCall is blunt: managing thyroid with only TSH is like “throwing darts blindfolded.” If you still feel awful, you may simply not have enough information on the labs to treat what’s actually happening.
2) The full thyroid panel everyone deserves
McCall’s “bare minimum” thyroid labs:
TSH
Free T4
Free T3 (the star of the show)
Reverse T3
TPO antibodies + Thyroglobulin antibodies (to assess Hashimoto’s)
3) Free T3 is your “gasoline” hormone
Her best analogy: Free T4 is crude oil—it must convert into Free T3 (gasoline) to power your body. Mood, metabolism, digestion, hair, skin, energy, cholesterol, blood sugar—T3 touches it all.
4) Reverse T3 can block your progress—even with “good” T3
You can have a pretty Free T3 number, but if Reverse T3 is high, it can block T3 from doing its job. Translation: you can look “fine” on paper and still feel terrible.
5) Hashimoto’s can improve—even go into remission
McCall separates the two clearly:
Hashimoto’s (autoimmune attack) can often be reduced and sometimes put into remission with lifestyle + targeted support.
Hypothyroidism (loss of thyroid function) usually still requires thyroid medication long-term because damaged tissue doesn’t magically grow back.
6) The 3 p.m. crash might be a dosing problem—not your willpower
If you’re taking T3-containing meds (Armour/NP/combination therapy), McCall explains these can peak and drop within hours. Her clinical approach often includes twice-a-day dosing (morning + mid-afternoon) to keep women from crashing right when they need to show up most.
